Alcohol Use Disorders Identification Test (AUDIT)

World Health Organization. A simple 10-question instrument to screen and identify people at risk of alcohol problems.


World Health Organization. A 3-item modified version of the AUDIT used to identify patients who are hazardous drinkers or have active alcohol use disorders.

CRAFFT Screening Tool 

Boston Children’s Hospital. A 6-question behavioral health screening tool to screen adolescents for high risk alcohol and other drug use disorders simultaneously. Recommended by the American Academy of Pediatrics' Committee on Substance Abuse for use with adolescents under the age of 21.

NIDA Modified ASSIST Drug Use Screening Tool 

National Institute on Drug Abuse. A short screening tool to assess a patient's risk level based on a Substance Involvement (SI) score. 

Online Screening Tool: Alcohol Screening

Join Together (a project of The Partnership at and Boston University School of Public Health). A short, confidential online screening tool which helps individuals assess their own alcohol consumption patterns to determine if their drinking is likely to be harming their health or increasing their risk for future harm.
